  • Excessive washing or scrubbing of the skin can aggravate the condition by irritating the skin. 
  • Touching and picking at comedones can cause irritation and infection spread.

Some skin products ( over-the-counter medications), but not all, can aggravate comedones by clogging pores, and greasy hair products (such as pomades) may aggravate acne.

Noncomedogenic or non-acnegenic over-the-counter medication/skincare claim not to clog pores. Oil-free and water-based makeup and skincare items can be less likely to cause acne.

****It is unknown if dietary factors or sun exposure improve, worsen, or have no impact on comedones. An ingrown hair, or a hair that does not emerge normally, may also clog the pore and cause a bulge or infection (causing inflammation and pus).

Genes can influence acne susceptibility. Some ethnic groups may be more inclined to comedones. For example, people of Latino and African descent people may have more comedonal inflammation, more comedonal acne, and an earlier onset of inflammation.

Comedonal Acne Breakout

Unlike the large, painful lesions that many people associate with acne breakouts, comedones are a type of non-inflammatory acne.

Both black and whiteheads are categorised as non-inflammatory acne — what you may call the building blocks of more serious acne breakouts.

Comedonal acne breakout can be usually treated by topical medications, such as retinoidsantibiotics (prescription-grade skincare), benzoyl peroxide, salicylic acid, azelaic acid and sulfur. These medications can be applied to the skin or taken orally.

Another treatment is using chemical peels to remove the top layers of skin and reduce clogged pores. In extreme cases, laser resurfacing may be used to smooth out bumps in the skin.

Comedonal acne can be tricky to treat and often requires a combination of treatments.

There is no evidence that sun exposure worsens comedonal acne, but it may be associated with the development of post-inflammatory hyperpigmentation.

Topical retinoids have been shown to prevent and treat comedonal acne. However, topical retinoids can cause skin irritation, redness, and peeling.

Antibiotics are commonly used to treat acne. Topical antibiotics are available as creams or gels applied directly to the skin. Oral antibiotics ( from your primary care doctor) may be prescribed for more severe cases of inflammatory acne that do not respond to topical treatments. Antibiotics kill bacteria on contact, but they do not penetrate deep into the skin or pores. Therefore limits their effectiveness in treating comedonal acne because the bacteria causing inflammation is inside the hair follicle rather than on top of it.

Benzoyl peroxide is a common treatment for comedonal acne because it kills P. acnes and reduces inflammation caused by the bacteria’s waste products (breakdown products). In addition, benzoyl peroxide aids in the removal of excess sebum from pores and helps prevent its buildup in hair follicles (follicular hyperkeratinisation).

Salicylic acid, an alpha hydroxy acid (AHA), is the frequent ingredient in over-the-counter acne treatments that is helpful against blackheads, whiteheads and other acne lesions and is a commonly used treatment for comedonal acne because it dissolves the proteins that bind dead skin cells to each other. As a result, these ingredients help loosen and remove dead skin cells from pores. It also helps prevent the buildup of excess sebum in hair follicles (follicular hyper keratinisation). In addition, it opens clogged pores to allow trapped oil, bacteria, and dead skin cells to be removed.

Benzoyl peroxide has been shown to decrease comedonal acne by killing P. acnes bacteria and reducing inflammation caused by bacteria’s waste products (breakdown products). 

Benzoyl peroxide also helps unplug blocked pores by loosening the shed skin cell protein plugs that clog pores. Salicylic acid has been shown to decrease comedonal acne by dissolving the protein bonds that bind dead skin cells together in pores, loosening and removing dead skin cells from pores, preventing the buildup of excess sebum in hair follicles (follicular hyper keratinisation), and opening clogged pores to allow trapped oil, bacteria, and dead skin cells to be removed.

Retinoids are medications that include tretinoin (Retin-A®) and adapalene (Differin®). These medications work by normalising the activity of the sebaceous glands and reducing the production of sebum. They also help unplug blocked pores by loosening the shed skin cell protein plugs that clog pores. They are considered first-line treatments for comedonal acne because they have been shown to reduce comedonal acne lesions faster than benzoyl peroxide or salicylic acid alone.

Antibiotics are used for severe forms of comedonal acne when other treatments have not worked well enough. The most common antibiotic used is doxycycline.
